Clients with Insurance Coverage
Saratoga Nutrition is accepting Blue Shield of Northeastern NY, CDPHP, Empire United Health Care, and MVP. This list will be updated periodically so to include more plans.
Confirmation of Insurance Coverage:
To be completed by clients:
1 - Verify with your insurance company that Mary Beth McCue is a Dietitian provider who is covered for "Medical Nutrition Therapy", or "Outpatient Nutrition Counseling", or any services under "Dietitian Services" that applies to your service.
2 - If needed, please verify: if you qualify for a diagnosis that is listed and covered, how many visits/year are covered, time duration covered per visit, co-payments that may apply, and ask if an MD order is required. If one is, this usually can be obtained by making a phone call to your primary care practitioner (PCP). If needed by your insurance, please bring this to your appointment.
3 - If you are covered, please e-mail or call Mary Beth with the above information. At that time, you will schedule an appointment, and a patient packet will be e-mailed to you. This must be completed and e-mailed back preferably 3 days or more prior to appointment.
Bring to appointment: Copy of your insurance card, and if applicable, co-pay in the form of cash or personal check made out to "Mary Beth McCue".
Clients without Insurance Coverage and paying cash:
Cash services are available at the rates below. Clients should plan on a minimal time of 60 minutes for the initial session. Most clients use 75 to 90 minutes at this session. Pre-pay is required at time of scheduling.

Consultation Rates: $28.00 / 15 minutes
Initial Session: 60 to 90 minutes: $112 - $168
Follow-up Session: 30 to 60 minutes, $56 - $112

A review of changes, what is working, symptom changes, eating habits, goals and more.
We accept pay-pal &/or (all) credit card payments via pay pal website. Please see below. You do not have to have a pay-pal account to use pay pal. You will be prompted to use a credit card of your choice.
Cancellation Policy: We require a 48 hour notice - which we have had an opportunity to respond to.
No Show: Client is charged the full session that they pre-paid.

Follow-up Package $250: Additional support sessions purchased in "twos".
